Managing life Stages

Managing the transition between different stages of life can be difficult as there may be new levels of care and assistance required from one stage to the next. Our life stage coordinator can work with the participant and their family to discuss a short term and long term support plan and develop a vision for a meaningful and happy life. We are able to handle the coordination of support services that are required, assist them while they are living at home and also encourage community participation.

Other areas of life stage planning include:

• Life transition planning which includes mentoring, peer support and individual skill development to help build confidence and independence
• Connecting and coordinating the right support services to each individual for their current life stage
• Ongoing assessment of support requirements, adding or subtracting support services when it may be required or if circumstances change
• Providing assistance and advice with daily planning, decision making and budgeting
• Assisting with accommodation and tenancy obligations
